6) Part 3 – Teaching Assessment Forms

 
The Teaching Assessment Forms will help you and your trainees manage the Teaching Assessment Sessions.
 

 
 
 
This is the standard Session Notes Form (as used at P2 too). Use this form to make notes if needed.
 

 
 
 
This is the standard Reflection Form (as used at P2 too). Use this form to allow the trainee’s to reflect on the session.

 
 
 
Each trainee will be asked to bring these documents to the session and use the forms to as they assess each other’s teaching when in the back of a 2:1 session.
 
This will form the basis of the reflection and work needed.
 
You can then use these forms to take a photo on your phone and upload.
